When: When can I request a referral for testing? Anytime!
Testing is administered Monday through Friday, 8:00 a.m. to 5:00 p.m.
Testing appointments will be scheduled with each client directly, and will be arranged within two to three weeks of a referral, depending upon availability.
Please remember to fax us a copy of the 0390R authorized codes and Claim Work Evaluation Paid-to-Date Amount from the Claim and Account Center to (360) 709-9806 on the same day that you make the testing referral. We will not be able to schedule the appointment until this information has been received.

Authorized codes are found in “What is covered under this claim?” under Treatment and Services. Look for the current 0390R codes and print this page to send to us.
Print the “Vocational Information page” which shows the Claim Work Evaluation Paid-To-Date Amount.
Aptitude testing reports will be returned to the Vocational Rehabilitation Counselor within five business days of testing administration. When completing the referral form, please be sure to specify how you would like to receive the report: via fax, email, or regular mail.
